4th Quarter Instructional Change Form
Beginning Monday, March 08, 2021, families will be given the opportunity to inform us that their child will be returning to in-person learning for the 4th 9 weeks, which begins on March 22, 2021.
Exiting/Entering Virtual Jefferson - If a family would like to change their child's instructional model, they should complete the 4th Quarter Instruction Form.
*Note – Only students requesting a change of instructional model should complete the form.
The online form can be found here and should be placed on your school’s website.
Families can complete the form online or complete a paper form and submit it to your school’s office in-person or via email
Deadline to submit a request to exit virtual is March 26, 2021.
