It is the responsibility of the parent/guardian to inform the school nurse about their child's health conditions (i.e., asthma, diabetes, ADHD, seizures, heart problems, sickle cell anemia, etc.) by completing the Jefferson Parish Schools Health Information Form annually.

If your child requires specialized medical procedures or close monitoring at school (such as diabetes care, seizure management, or tube feedings), please contact your child’s school nurse directly as soon as possible.

You will need to schedule a pre-school appointment to discuss your child’s medical needs, review care plans, and ensure all required paperwork is completed before your child can safely start the school year.

Meal service to students with special dietary needs is provided to all school sites. Federal and
State regulations require a completed diet prescription form for any type of change or
substitution to the student's diet. The special diet prescription form will need to be completed by
your child’s physician or medical authority and returned to your school cafeteria. Parents will
need to provide any necessary meals for student until the form is processed. If you have

Medicaid Reimbursement for Schools

The Louisiana Department of Health (LDH) Medicaid program allows school districts to request reimbursement for costs associated with providing certain health services to students insured with Medicaid.

If your child receives services through an Individualized Education Program (IEP) or an Individualized Health Plan (IHP), Jefferson Parish Schools can seek reimbursement for:

• Physical and Occupational Therapy

• Speech Pathology

• Behavioral Health Services

• Nursing Services

• Special Transportation

Before we can access these Medicaid benefits or share any related personally identifiable information, schools are required by law to provide notice and obtain written parental consent.

Jefferson Parish Schools seeks parental consent for every student to help secure this valuable funding, which goes directly back into supporting our school programs.

• Completely Voluntary: Giving consent is 100% optional.

• No Penalty to Your Child: If you choose to deny or withdraw your consent, your child will still receive all of their required school services. Your child's school-based care will never be affected or penalized.

• Change Your Mind Anytime: You have the right to deny or withdraw this consent at any time.

Stock Epinephrine 

Every school makes every effort to stock at least two (2) doses of unassigned, emergency auto-injectable epinephrine (EpiPens).

Under a doctor's standing order, a school nurse or a trained school employee can administer this emergency epinephrine to any student on school grounds during the school day if they believe, in good faith, that the student is having a severe allergic reaction—even if that student does not have a personal prescription.

• This does NOT replace personal medication: This emergency stock is a backup safety net. It does not override or replace student-specific doctor orders. If your child has a known allergy, you must still provide their personal, prescribed EpiPen to the school.

• School Premises Only: This policy only applies on school grounds during the normal academic day. It does not extend to off-campus activities, school transportation, or field trips.

Students at High Risk for Anaphylaxis

If your child is at high risk for a severe allergic reaction (anaphylaxis) but is unable to self-administer their own EpiPen, Jefferson Parish Schools allows a personal supply of auto-injectable epinephrine to be safely stored right in their assigned classroom(s).

To set up this classroom accommodation, parents or legal guardians must provide the school with the following items:

• The Medication: The supply of auto-injectable epinephrine to be kept securely in your child's classroom.

• Physician's Written Authorization: A signed order from a Louisiana-licensed medical provider.

• Physician's Certification of Capability: A written statement from the doctor confirming that your child is at high risk for anaphylaxis and is not capable of self-administering the medication.

• A Written Treatment Plan: A specific, step-by-step emergency care plan from your healthcare provider for managing an allergic episode.

Stock Naloxone (NARCAN)

To protect the health and safety of our students, staff, and campus visitors, Jefferson Parish Schools maintains an emergency backup supply of Naloxone (NARCAN Nasal Spray) at each school facility.

Under Louisiana state law, trained school personnel are authorized to administer NARCAN Nasal Spray on campus to any student, staff member, or visitor who is believed or suspected to be experiencing an opioid overdose.

• School Premises Only: This emergency supply is strictly maintained for situations occurring on school grounds during the school day.

• No Off-Campus Coverage: This policy does not extend to activities off school grounds, including school transportation, bus routes, field trips, or away-from-school athletic events.

To keep your child safe and healthy, communication is key! It is the parent's or guardian's responsibility to ensure the school nurse has all the information and supplies needed to care for your child.

1. Tell Us About Your Child’s Health Conditions 

If your child has a medical condition (such as asthma, diabetes, ADHD, seizures, heart problems, sickle cell anemia, etc.), you must inform the school nurse. You can do this in one of two ways:

• Complete and submit the JPS Health Information Form.

• Schedule a personal health conference directly with the School Nurse.

2. Submit Authorization for Daily Accommodations 

If your child requires special health accommodations or procedures during the school day (such as blood sugar checks, catheterization, tracheostomy care, or G-tube feedings), the proper paperwork is required before services can begin:

• You must submit a State of Louisiana Physician’s Authorization for Specialized Care form completed by a Louisiana-licensed physician.

3. Complete a Release of Information Form 

To coordinate the best care possible, our health team may need to speak with your child's doctors.

• In order for the school to send or receive medical information to or from a physician or facility outside of Jefferson Parish Schools, the parent must complete a Release of Information Form.

4. Provide All Medical Supplies and Equipment 

• It is the parent’s responsibility to provide all necessary equipment, medication, and supplies required for any medical procedures performed at school. The school does not provide these medical supplies.

To keep our schools healthy and safe, all students registering for school in Jefferson Parish must be immunized according to the Louisiana Department of Health Office of Public Health schedule.

• You must provide a State of Louisiana Universal Certificate of Immunization to verify your child's vaccines and complete their school enrollment.

• The Right to Dissent: A parent or legal guardian has the right to decline these immunization requirements.

• How to Opt Out: To do this, you must complete and submit an official Statement of Dissent from Immunizations form.

Please be aware that if a vaccine-preventable disease outbreak occurs at your child's school:

• Following the recommendation of the Office of Public Health, school administrators may exclude unvaccinated students from attending school.

• This exclusion will remain in place until the disease's incubation period has fully passed, or until acceptable evidence of immunization is provided.

• The health and safety of your child is our absolute priority. Here is how Jefferson Parish Schools handles health situations during the school day:

  For Minor Injuries

• The School Nurse or a trained, unlicensed Jefferson Parish Schools employee will provide standard first-aid care.

• A school staff member will notify the parent or legal guardian about the situation.​​​​​​

  For Life Threatening Injuries

• Emergency Medical Services (911) will be called immediately to provide urgent care.

•  The school will contact the parent or legal guardian immediately after calling emergency services.

• To ensure we can reach you instantly if your child becomes sick or injured, all parents and guardians are required to complete or update the Student Emergency Card every school year.

Students with food allergies or special dietary needs should submit a Medical Statement to Request School Meal Modification completed by a Louisiana licensed physician to the school nurse.

The following form is required to be completed by the physician annually for a student's diet to be modified:

Anaphylaxis is a severe, rapid allergic reaction that can be life-threatening. In compliance with Louisiana state law (R.S. 17:436.1 (K)), Jefferson Parish Schools follows strict guidelines to protect both students with known allergies and those who may experience an unexpected, first-time allergic reaction at school.

For Parents of Students with Known Severe Allergies

If your child has a known, life-threatening allergy (to food, insect stings, latex, etc.), you are strongly encouraged to notify the School Nurse immediately.

Every school year, you must provide the school with:

• Written emergency instructions from your child’s healthcare provider.

• Your child's prescribed emergency medications (such as an EpiPen).

How We Keep Our Campuses Safe

To ensure a rapid and safe response to allergic emergencies, Jefferson Parish Schools implements the following district-wide protections:

• Staff Education & Training: School personnel receive regular training on how to manage life-threatening allergies, including how to properly administer emergency epinephrine injectors.

• Emergency Response Procedures: We maintain clear, step-by-step protocols for responding to both known allergic reactions and unexpected, first-time emergencies on campus.

• High-Risk Management: We use a structured process to closely manage and monitor students who are at a high risk for anaphylaxis.

• Allergen Exposure Prevention: Our schools follow strict protocols and precautions to reduce and prevent student exposure to common food and environmental allergens.

JP Schools School Nurses conduct routine vision and hearing screenings every year for all students in Pre-K, Kindergarten, 1st, 3rd, 5th, 7th, and 9th grades.  Students in other grades may also be screened at the request of a parent or school staff, if a problem is suspected, or if they are being evaluated for special education services.

If a student fails his/her vision or hearing screening, the nurse will send  a letter to the parent/guardian. Evaluation by a LA licensed eye doctor/audiologist is strongly encouraged for all screening failures.

A copy of the doctor’s evaluation or prescription should be returned to the School Nurse.

In alignment with CDC Respiratory Virus Guidance, COVID-19, Flu, and RSV are managed under the same timeline:

1. Stay Home & Notify: If your child tests positive, keep them home and notify the school principal and nurse.

2. When to Return: There is no mandatory 5-day isolation period. Your child can return as soon as their symptoms have improved and they have been completely fever-free for at least 24 hours (without fever-reducing medicine).

3. Next 5 Days: Upon returning, it is highly recommended that students wear a well-fitting mask, practice physical distancing when possible, and practice frequent handwashing.
