Instructional Model Change Request
Exiting Virtual Instruction
Students requesting to move FROM Virtual TO In-Person Learning, during the 4th Nine Weeks Marking period, must complete a School Instructional Model Change Request Form.
Once the form has been completed, signed by the parent and identification provided, the School Principal/Designee will review the form, sign it and forward it to the Compliance Office, within 48 hours of the parent submitting the form to the school.
Once a decision has been made, it will be communicated to the Principal/Designee and Secretary and the school will inform the parent of the decision rendered.
Upon review of the request, parents are sometimes requested to provide supporting documentation, which may delay the rendering of a decision.
