Consent forms for the Behavioral Health SBHC social worker. In order for a student to be seen by the SBHC staff we have to have a signed consent form.

The new School Based Health Center at Chateau Estates offers families and children the convenience of behavioral health services on school campus with no out of pocket expense. The Health Center enables students to stay at school and parents to remain at work. Individual and group therapy sessions to address issues with stress or anxiety, social skills, sadness and grief, anger and attention management, self -esteem, and motivation are offered. Psychiatry services are available by appointment and prescriptions can be filled through the Access Health Care Pharmacy and delivered at no cost.
